What is the best software/hardware combination to do home video conferencing (DSL to DSL)at low cost?

I used Net Meeting & Yahoo! Messenger video to do home video conferencing but the quality is very low, I am willing to invest in a new WebCam, but I need to have a better software. Any recommendation?
